- High consequence: changes to treatment, triage, discharge, medication, or escalation require independent professional validation and explicit approval before action.

Complete a Care Handoff Integrity Packet with outcome, visible assumptions, evidence and date, validation status, Prepared By, Validated By, Approved By where required, and residual risk.
Approved-use boundaries
